Care at Heart grew from Heart of Hospice, a progressive hospice with a vision to transform end-of-life care in the communities we serve. Since 2007, Heart of Hospice has been compassionately providing quality, hospice care throughout the Southeast.
We have proven our commitment to realize this vision through the hospice services we deliver and believed it was time to expand our care further upstream, enabling us to help many more patients and their loved ones who desire support.
Care at Heart, the parent organization for Heart of Hospice and all our other service lines, is the next step in realizing our persisting vision: to transform the end-of-life care continuum in the communities we serve. By providing care earlier in a patient's illness progression, we grant access to care for many more patients and their loved ones.
The most common regret we hear from patients and their loved ones is that they wish they had elected our services sooner. Care at Heart allows us to better fulfill our vision of caring for patients during more than just the final few weeks of their lives by guiding them through their entire advanced illness journey.
At Care at Heart, we value diversity and believe all individuals have the right to be themselves. We meet our patients and our families where they are, irrespective of their beliefs, lifestyle, diagnosis, DNR status, care plan choices, or desire to hold hope. We know that transforming end-of-life care begins by listening to and accepting those who need it.
